New Patient Information

North Florida Dermatology Associates would like to welcome you! Please print out a New Patient Welcome Packet, fill out all the forms, and bring them with you to your appointment. You will also need for your visit your insurance card, driver's license, and any copay or deductible that you are responsible for. Copays and deductibles will be collected when you check in. We will also need a list of any medications you are currently taking. You may bring these medications with you if you would like. In order to expedite your visit, it is important that you come to your appointment 30 minutes early and bring all the above items with you. If the appointment is for a minor child, please have available the child's Social Security card or Social Security number.
